How do I add or drop a dependent or domestic partner to my medical and dental insurance?
Changes to dependents on your plan are only allowed during the annual open enrollment period for medical and dental coverage. Open enrollment occurs in June of each year. Dependents are same or opposite sex spouses and children under the age of 26 for medical coverage; children under the age of 24 for dental coverage.
During the Open Enrollment period, you can add a dependent or domestic partner by completing a new medical or dental enrollment form and an election form:
- Blue Cross Application
- Blue Cross Dependent Child Application
- Delta Dental Application
If you would like to add a same or opposite sex domestic partner to your insurance, you must also complete a Declaration of Domestic Partnership form, which is reviewed and approved by the insurance company. More information is available at Same-Sex Spouses & Domestic Partners.
Outside of the Open Enrollment period, dependents may be added or dropped when certain events occur. You must notify Human Resources within 60 days of a birth, adoption, marriage, divorce or your dependent’s loss of coverage in order for a dependent to be added or dropped from coverage.
If a dependent loses coverage, they must provide a HIPAA certificate from their former insurance company before they can be added to your coverage.

How do I apply for a leave of absence?
You should first advise your supervisor that you will be requesting a leave and then complete the appropriate leave of absence form. An employee who is out of work for 3 days consecutively or who will be out periodically due to their own or a dependent’s medical condition must complete a Family Medical Leave of Absence Request Application and submit a completed Health Care Provider Certification form.
These forms should be completed and returned to Human Resources at least 15 days before your requested leave start date, or, in case of an emergency situation, as soon as reasonably possible. All leaves are reviewed for approval by the University.
For a full list of leaves and leave qualifications, please refer to the Policies page. If you are a union employee, please refer to your contract as well.
